Pruritus ani is an intense chronic itching in the anal region. Usually, it begins gradually and becomes a self-perpetuating itch-scratch cycle.(14)

What are the causes of pruritus ani?

Pruritus ani is a common symptom that can be associated with a wide range of conditions:

  • Diabetes, liver disease, thyroid disease
  • Depression, anxiety disorders
  • Fungal, bacterial and viral infections
  • Allergic reactions eg, contact dermatitis caused by local anesthetics, such as "-caine" preparations
  • Antibiotic treatment
  • Chronic constipation, diarrhea, anal incontinence
  • Local anal disorders: anal fissures (pruritus ani accompanies anal fissures in 50% of cases),(15,16) hemorrhoids and cryptitis. Only hemorrhoidal tags that are inflamed or associated with poor hygiene produce pruritus.(1)
  • Ingestion of certain foods, especially spices, coffee, beer, cola and medications
  • The perianal area can be highly sensitive to perfumes, soaps, clothes, fabrics, excessive sweating and superficial injury
  • Hygiene, either poor with residual irritating feces or over meticulous with excessive use of soap and rubbing also may be associated with pruritus
